Continuity of care when moving out of Camden

Under the Care Act 2014, if you are receiving care and you plan to move to a new area, you must inform your new local authority (council) as soon as possible. A number of steps may then be followed.

This second council (the place you are moving to) will then contact your current council to confirm your plans and to obtain your care and support plan information so that they understand what support you may currently be receiving. The second council may also request other information, such as a financial assessment. This may help to ensure that you receive uninterrupted care and support upon your arrival.

The second authority may also take steps to make sure that you really do want to move to their area by speaking to you directly.

If you have a carer, who will continue to care for you after your move, the first authority must also provide a copy of the carer’s support plan.

Once the second council has received all the required information, then they must conduct their own assessment of your needs and if a carer is also moving with you, then they must also assess the needs of the carer as well. Both assessments can take place before you move to your new area.

Your needs may change when you move home, for example if you are nearer to family, then you may have more sources of support. Alternatively, being in a new place may mean that you have new needs. Therefore, it is vital that the new local authority assesses you, to help ensure that the right care and support is in place.

If the second authority finds any needs which are different to those identified by the first authority (and in the care and support plan or most recent assessment provided), it must explain to you in writing why this is the case.

The first authority must continue to stay connected with the second authority, to keep track of progress, and it must also keep you involved in this part of the process.

On the day of your arrival to your new boroughThe process above should mean that the council area you are moving to knows about you and what support you may draw on and have services to meet these in place when you arrive.

If you move and the council has not yet carried out an assessment, the second authority must meet any needs that were being met by the first authority. This also applies to the needs of any carers. The second authority will use information from the first authority to decide what this is. This will continue until the second authority has carried out its own assessment, and put in place care and support on the basis of this assessment.
