Camden Health Improvement Practice (CHIP) for Homeless Patients
Camden Health Improvement Practice(CHIP) offers a GP surgery and primary care services for homeless people, including:
- Patients who are street homeless, in hostels, B&Bs or sofa surfing
- Patients with substance misuse and/ or alcohol and mental health problems
- Refugees and asylum seekers
- Victims of modern slavery,
- Patients who have been repatriated from abroad once their prison sentence has been completed.
We offer:
- Full registration Substance misuse clinic
- Spirometry clinic
- Sexual health clinic
- Hepatology clinic (which includes initiation of Hep C treatment)
- Welfare/benefits clinic
- Tissue viability clinic for patients with complex ulcers
- Team Around the Practice
- Social Prescribing and Care Navigator service for escorting chaotic patients to hospital appointments/practice appointments
- Screening for BBV
Please note that we are unable to offer services for children or adolescents.
Referral to the services is open and by drop-in and we run 7 clinics each week. Patients need to present themselves at the beginning of the clinic and they can register with the Nurse and see the GP if necessary. There are 3 GP sessions that hold booked appointments each week, so patients can make an appointment to register.
