Going into and coming out of hospital

If you are expecting a planned stay in hospital there is advice and information available on the NHS Choices website to help you prepare for going into hospital as well as advice on being discharged from hospital.  Your benefits may be affected.  For more information on this visit the Independent Age website.

Support is available for you at home when you return from hospital; go to the Care options section of this website for information on types of care and support available and also the Recovering from illness or accidents section which also provides details about short term care (reablement) when you come out of hospital.

If you need equipment organised for you for when you leave hospital you may want to contact the council's Occupational Therapy service, more information is available in the Equipment and technology to help you section, or you can discuss with your District Nurse.
